Home health and hospice services available to those living in Washington, Ozaukee, Sheboygan, Fond du Lac, Dodge and Waukesha counties.Home medical equipment (durable medical equipment) delivery / service area includes four southeast Wisconsin counties: Milwaukee, Ozaukee, Washington and Waukesha.The nation's first to provide specialized care for those facing Alzheimer's disease and related dementia / memory loss in 1976. A brand new state-of-the-art assisted living for those with memory loss -- The Cottages at Cedar Run -- is scheduled to open in July 2014. The main campus is located at 5595 County Road Z, about two miles south of Highway 33, above Big Cedar Lake in West Bend, WI, with additional locations in Elkhart Lake (just north of County Road A off Highway 67) (Rhine Street and Lincoln).
